加強院前治療中毒病人服務 Enhancing pre-hospital poisoning consultation and treatment services

  醫院管理局轄下香港中毒諮詢中心聯同消防處將由十一月一日起在全港推行「院前中毒諮詢及活性炭治療中毒病人服務」,爭取在最短時間內為中毒病人提供最適切的治理。

   香港中毒諮詢中心總監劉飛龍醫生今日(十月二十七日)在記者招待會上公布這項服務計劃時表示,在過去半年的先導計劃,香港中毒諮詢中心合共處理了二十二 宗院前中毒諮詢個案,大部分為服用過量或誤服藥物。劉飛龍說︰「全面推行這項服務,有助提升中毒病人的生存機會,在試驗期間亦沒有發現嚴重併發症。估計每 年將有超過二百名中毒病人,可以在中毒後最關鍵的黃金一小時內,提早得到治療。」

  這項服務的先導計劃於今年三月至九月期間在消防處的港島東及九龍東救護分區試行。消防處救護總長梁紹康表示,是次試行計劃成效超卓,雙方溝通十分暢順,全面推行此計劃將大大提升院前救護服務質素。

  劉飛龍指出,本港是全球首個地區安排受過毒理學培訓的醫生24小時當值,為中毒病人作院前評估及提供電話支援服務。設於基督教聯合醫院的毒理學培訓中心,定期為醫護人員舉辦課程,提升醫護人員在救治中毒病人時的專業水平。

  而消防處於去年十一月至今年九月的持續醫療教育課程中,亦已為1,091名急救醫療助理提供有關訓練。課程由消防處醫務總監設計,內容包括中毒簡介、活性炭使用、治理程序、適用症、禁忌症及個案討論等。

   梁紹康說︰「根據新服務的安排,日後所有救護車會配置活性炭解毒劑,消防處急救醫療助理在處理中毒病人時,會先為病人作出全面的檢查、錄取維生指數及提 供合適的基本治療。其後他們會收集吞服物質的資料如:物質的名稱和特徵、吞服數量等,再依據消防處及香港中毒諮詢中心共同制訂的指引,透過電話熱線即時聯 絡當值醫生,醫生會為中毒病人進行即時評估,再指示急救醫療助理為合適的病人提供活性炭解毒劑作初步治理然後轉送醫院急症室跟進。」

The following is issued on behalf of the Hospital Authority:

Starting November 1, the Hospital Authority's Hong Kong Poison Information Centre (HKPIC) and the Fire Services Department (FSD) will implement the "Pre-Hospital Poisoning Consultation and Activated Charcoal Treatment Service" to ensure correct and appropriate treatment is promptly given to poisoned patients.

At the press conference today (October 27), Director of HKPIC, Dr Lau Fei-lung, said that the HKPIC handled 22 pre-hospital consultations during the six-month pilot period. Most patients had ingested drugs, either accidentally or with suicidal intent.

"No complications were observed and the condition of poisoned patients was significantly enhanced. As the hour immediately after poisoning is the most crucial moment in rescuing a patient, the extension of the service territory-wide would help more than 200 patients a year to get the antidote within an hour," Dr Lau said.

The service was piloted in the Hong Kong East and Kowloon East sub-division of FSD Ambulance Command from March to September 2010. Chief Ambulance Officer of FSD, Mr Leung Shiu-hong, said that the result of the trial scheme was remarkable. Communication between both parties was very smooth and the extension of the service territory-wide would enhance the quality of the pre-hospital ambulance service.

Dr Lau said that Hong Kong is the first city in the world having toxicology-trained doctors on duty providing 24 hours pre-hospital poisoning assessment and telephone-support services. The Toxicology Training Centre, located at the United Christian Hospital, organises training regularly to enhance the professional standard of healthcare professionals when dealing with poisoned patients.

FSD provided relevant training to 1,091 paramedics in the Continuing Medical Education course held from November 2009 to September 2010. Designed by the Medical Directors of FSD, the course provides an overview of poisoning and covers the use of activated charcoal, treatment protocol, indications, contra-indications and case studies.

"According to the new arrangement, all FSD ambulances will be equipped with activated charcoal. Paramedics on site will firstly conduct a comprehensive assessment and provide suitable basic treatment to poisoned patients. After collecting information on substances taken, they will contact HKPIC doctors immediately for a preliminary assessment according to a standard protocol set by the FSD and HKPIC. Instruction will be given to the paramedics to give activated charcoal to poisoned patients, if appropriate, as a preliminary treatment before sending them to hospitals," Mr Leung said.
